There is no registration fee for the symposium or the evening reception but the number of attendees is limited. Moreover, we need a precise count of guests for the catered dinner seating. Please fill out the form below indicating the event(s) you will attend. One form per participant.

• • If you are coming with a guest, please fill-in a separate form for each guest, but provide your email address as contact information

• If you need to cancel your reservation, please use this form as well and choose the appropriate "Regrets" option

  • Once you submit the form, your name and the chosen event (but NOT your email and comments) should appear on the Guest List page in 5 to 10 minutes
